Check whether the scanned copies of the following documents are uploaded along with the applicaiton. (a) Online Transaction Receipt of Registration and Inspection Fee paid for the certificate course (b) Registration Certificate of Society or Trust (c) Rental Deed or Ownership Document of Institute Building (d) Plan showing the furniture layout of each room prepared by an architect (e) Internal and External Photographs of Institute Building with Date, Institute Name and Address (f) Faculty Members' Qualification Documents and Appointment Letters (g) Lab Equipment, Machinery and Tools Purchase Bills or Delivery Challans (h) Books Purchase Bills or Delivery Challans (i) Previous Year's Affiliation Letter, if have. (j) Declaration of the Applicant : Download (k) Undertaking of the Head of Institution or Organization : Download (l) Duly signed in Check List of the documents uploaded : Download |

Fill all the fields related with the course(s) being applied, building, staff, equipment and library. |
| 3. |
Take a printout of this application after submission and present it to the inspecting officer appointed by the board. |
| 4. |
The institution should be run by a registered Society / Trust. |
| 5. |
Affiliation will be issued to the course, if a minimum of ten (10) candidates are admitted in the each batch of that course. |
| 6. |
Registration Fee / Inspection Fee once paid will neither be refunded nor be adjusted under any circumstances. |
